Along with the form, you will need to include a Declaration by the Applicants (one from the husband and one from the wife), proof of income, one family photo, names and contact telephone numbers of two referees, and a chest X-ray report of the female applicant (the primary carer).

We at Mother's Choice will conduct an interview and home visit with you and your family and prepare your application for the Central Foster Care Unit of the Social Welfare Department.

When the application is accepted, you will gain approval to become foster parents. You will then be invited to attend pre-service training before commencement of foster placement.

We at Mother's Choice will introduce you to the child before placement begins and follow up with monthly supervision and support.
